ATTENTION NEW PATIENTS:
As required by law, we must validate the identification of all patients establishing care with our practice. Please bring photo identification (ex: Drivers License, Passport, Military ID, School ID etc.) and your health insurance card with you to this appointment.
- You will be asked to electronically sign our Notice of Privacy Practices, Cancellation/No Show Policy, and Health Information Request form. A copy of these are available upon request.
- You will be having blood work drawn twenty minutes prior to your office appointment.
- You do not have to fast for the blood work.
- Bring ALL medication bottles or a complete list of all medications, including vitamins and/or any herbal supplements that you are currently taking.
- Co-payments, co-insurance and deductibles are due at time of service. For your convenience we accept VISA, MasterCard, ATM/Debit cards as well as cash and personal checks.
